I thought this incident happened over Alaska not Indonesia. I remember a story
about a 747 encountering volcanic ash over Alaska causing all 4 engines to shut
down.  As the ash cooled it came off the fan blades and the pilots were able to
restart then engines.  Perhaps it was KLM.
