[PATCH 0/8 v7] Unprivileged SG_IO support

As a result of RFC [1], this implements the unprivleged SG_IO
support.

v6 - v7:
   * No restoring of unpriv_sgio per Daniel's thought.
   * Use "major:minor" as the hash key per Jirka's suggestion.
