ToC note (still is Re: Wiki page structure [RFC])

I forgot to mention that in these guidelines, I added a requirement for
a [[TableOfContents]] macro at the start of every page.

There may be disagreement here.  My reasons for making this a rule are:

* It is common practice and is not confusing to people
* Common in other Wiki (such as MediaWiki), so it is somewhat expected
* Easy to reproduce in XML with a <toc> in each <chapter>
* It really makes for a better reader experience

There are exceptions to most rules I wrote down, so please read the
