I'm guessing it's the latter, since the former is explicitly not a
package manager, but I'm not sure what makes you think this extension is
more secure than Emacs' package management. It fetches a file from a
URL, unpacks it, and installs it. There's no signature checking or
anything.
If that's trustworthy, then so is "wget
