Failure '?' very often (always?) is an indication that the test could not
be performed because of failure to access the file. Failure 'S' is typical
for config/data or regularly recompiled/optimized files, for example.

These are just some of the issues.
After that I started by running rpm -V <package>
yum reinstall <package> if it failed.
Then rpm -V <package> to verify.
I will check a different machine as well that I upgraded but it was from
20 to 21 and 32 bit.
