Looks like this was maybe part of 20_linux_xen, according to /etc/grub.d/README but it's not there now. It may be easier to just google something for memtest grub, and then modify 40_custom to contain that entry, then run grub2-mkconfig -o /boot/grub2/grub.cfg to add it to the grub.cfg. The gotcha is this assumes BIOS. On UEFI, the memtest86+ binary is completely different, and the grub menu entry needed is also going to be slightly different.
