I have had this happen once in a long while. I end up killing the
window in focus (ctrl-d) and it seems to go back to normal after that.

Is this not a possibility?

Whatever works. I don't remember, but maybe once or twice I was able to regain control of the desktop, that way. But, more often than not, kicking X was the only way out.
