I've run alsamixer -c0 and all the levels are set to their maximums.

I've removed pulseaudio and mplayer and removed the .mplayer folder and
then reinstalled mplayer.

How should I proceed from here ?

I've tried it and it works. You also get pamixer which can show you whether or not PA sees your audio card. Unfortunately, I have a different Intel chip than you, mine is the AD198x.
