I see this as well, and have even developed a hypothesis as to when it happens: I right click on a link, and use "open in new tab" Then I move the mouse down a bit and right click on a new link to bring up the menu so I can do another new tab. But instead of brining up the menu again, it acts exactly like the previous menu is still posted and I clicked on the old menu's "bookmark this link" item which was (when the menu was up) physically located over the new link I just click on.
