- backup the /var/lib/rpm/
- reboot with a rescue cd (or boot entry).
- use the rpm from the rescue mode to try the rpm with alternate root
rpm --root=/mnt/sysimage

You wouldn't happen to be using ext4 as the filesystem /var/lib/rpm is on?
That aside, the good ole "rm -f /var/lib/rpm/__*" should do the trick as
long as there are no other rpmdb users at the time (which is harder to
ensure these days with so many things accessing the rpmdb).
