Normally I find that the first time you click on the Kmix icon > the volume control is put in the panel. Have a look in Kmix's "settings" > "configure-kmix". There should be a line "Enable system tray volume > control", which needs to be checked. Thanks very much. Going to K=>Multimedia=>KMix and Settings=>Configure KMix clicking on Apply brought the speaker icon into the panel.
