So does it start okay, or does that fail because there's a lock file? You might have to restart the subsystem, or even manually remove the lock file and then start the subsystem. If it's dying there should be something in the logs.

I have found that even restarting it after awhile it will fail again with the "irqbalance dead but subsys locked". It seems that if you have less than two cores or only one cache domain than you will not be able to benefit from the irqbalance service.
