Are you saying that turning:
browser.download.hide_plugins_without_extensions to false does not allow
you to do what you want in the Download window in preferences?
Because that allows me to do just what you  is what you want to do. 
However, there is a cost. If you associate a file type with a program
then when you open it you first have to answer a question as to if using
that program is what you want to do, then you get a download window,
which in some cases requires you to click open to have it done. What a
pain.
