Drop the resulting info into bugzilla, and we might stand a fighting chance of getting to the bottom of it. Right now, I don't know whether this is a kernel, mkinitrd or device-mapper bug. Alternatively, if you have a second machine, and a serial cable, booting the buggy machine with console=ttyS0,38400 and setting up a minicom session on the other end will make it even easier to capture to a logfile.
