hi David

fw was my first a thought, as i saw 2.4 !

just now i fired up my hp nx6110 lappy: 
ipw2200BG rev 05 (pciid: 8086:4220;sub: 103c:12f6)
livna firmware, 
kernel 2174

without connected to my AP (it's off), i can scan my neighbors AP by a quality: 73/100, signal level: -55dBm in manged mode.
and the building is maid with strong concrete. difficult to drill holes in.

btw. i maid the experience that i get the best connection to *MY* AP, if i choose the same channel like my neighbor.
(same AP manufacturers)

maybe it's useful to check:

- *only* the last firmware in the directory and modprobe -r /-sv ipw2200
  (i got some strange experiences with fw-download with another card; 
   means i'm not sure if modprobe -r unloads and -sv loads the firmware; 
   should do, but since yesterday i'm really not sure !)

- udevmonitor 
- iwevent
- your AP: no "turbo's" aka nitro,..., 
- wep/wpa

that's what i would do, but ...
