Cross-posting w/o even mentioning the other mailing lists in the Cc or

In general (not only mythtv related):

o Get the unpatched src.rpm
o "install" it (rpm -ihv foo.src.rpm)
o See where the sources and specs land (by default under
  /usr/src/redhat). Modify these locations as required (for example
  for building as non-root and /usr/src/redhat not writable by
  non-root, which is the default) in ~/.rpmmacros
o Add a patch to the sources and the specfile
o rpmbuild -ba /path/to/specfile/foo.spec
