Or merely displaying one? I would expect okular to display the document in whatever paper size it was created with. For the print dialog, though (which is not specific to okular) I have seen an issue where it doesn't use defaults from the print system (including paper size). There is a copr at https://copr.fedorainfracloud.org/coprs/kkofler/qt5-qtbase-print-dialog-advanced/ which has a backport of the print dialog functions from a newer version of Qt. It did seem to help with various Qt printing issues when I tried it.
