Make sure cmsfscat makes it in to the s390x initrd, it lives in
/usr/sbin after upd-instroot has run.

Thinking of
https://bugzilla.redhat.com/show_bug.cgi?id=522535#c3
I'm not sure, if KEEPFILE in upd-instroot might need to be tweaked as
well since it says:
sbin/cmsfscat
while the file according to the RPM package actually lives in:
usr/sbin/cmsfscat
which is also the path that you seem to reference correctly in mk-images
here.

You sure? This is what I see in s390utils-cmsfs:

$ rpm -qlp s390utils-cmsfs-1.8.1-3.el6.s390x.rpm /sbin/cmsfscat
/sbin/cmsfsck
/sbin/cmsfscp
/sbin/cmsfslst
/sbin/cmsfsvol
/usr/share/man/man8/cmsfscat.8.gz
/usr/share/man/man8/cmsfsck.8.gz
/usr/share/man/man8/cmsfscp.8.gz
/usr/share/man/man8/cmsfslst.8.gz
/usr/share/man/man8/cmsfsvol.8.gz
