The upd-instroot just has mke2fs and mkfs.ext2 listed as keepfiles, and not mkfs.ext3. rpm2cpio apparently extracts the names but the actual file with the data is in mkfs.ext3 -- all three files need to be on the to-extract list, and then suddenly everything works fine.
