The order of the cards is independent of the order of the devices.

The cards are ordered this way because the internal GPU happens to have
the smaller number (as shown in the list output by lspci).  The devices
of a card are ordered this way for historical reasons.

Anyway, it is not really possible to choose a default output device
automatically.  It has to be configured somewhere, and your problem
appears to be that there are multiple sound system that each have their
own configuration.
