[Please note that I run Debian unstable "Sid", so I don't know about the 
availability of packages realtime-lsm and realtime-lsm-source in stable]

To enable realtime in your customised Debian 2.6.18 kernel, you need to 
set under security options:

CONFIG_SECURITY_CAPABILITIES=m

install the packages

realtime-lsm
realtime-lsm-source

and build the realtime-lsm module by unpacking the 
/usr/src/realtime-lsm.tar.bz2 and building and installing it.

I go to my kernel source directory and run

make modules-image

then run dpkg -i /usr/src/realtime-lsm-module-2.6.18*.deb

Check the documentation in /usr/share/doc/realtime-lsm/README.Debian for 
expanded details.
