Does the entry cn=SMD5,cn=Password Storage Schemes,cn=plugins,cn=config 
exist in your /etc/dirsrv/slapd-instance/dse.ldif?

If the entry is already in dse.ldif, when it is loaded, the plugin code 
will add nsslapd-pluginDescription - this doesn't happen when adding 
over LDAP.
